To be seen at the Stad Center, you need a referral from your child's primary care physician or from the specialist overseeing your child's care.
Here's what to do:
1. Send our referral form to your child's doctor
Send the doctor a link to our Stad Center referral page or download the referral form (PDF) yourself and pass that along.
When the referral form is complete, your child's physician should fax it to us following the instructions on the form.
2. Wait to hear from us
Once we have the referral, we'll contact you to set up an appointment. If you don't hear from us soon, follow up with your child's doctor to ensure the referral was completed.
